Become a Member
One of the most common questions we hear at the CRPHO is "How do I join?"
There are two easy steps to become part of the CRPHO.
- The first is to complete the CRPHO application form and submit the application fee to the CRPHO. To request the CRPHO application form, please click on the "Credentialing Request Form" button on the the left of this page. The electronic application will be sent directly to the email address of your choice. Application fees are dependent upon facility or specialty. Please contact the CRPHO at 319-366-5282 ext. 1 for current application fees.
- Determining your eligibility for equity ownership or membership status is the second step. Equity ownership is reserved for M.D.'s and D.O.'s. An equity purchase is a one-time investment. All other providers access CRPHO services as participating members. Members pay annual dues.
